17. Participation Requirements: Recreation, Gym, Physiotherapy or Exercise Physiology
17.1 If a guest would like to participate in the recreation program, they must:
• Fill in the medical history form which is part of the booking form. This form must be filled out each time a guest stays at Sargood on Collaroy. The form will be reviewed by Sargood clinical staff and if they think it appropriate the guest may be asked to produce medical clearance from their treating physician before participating in the program.
• Sign the terms and conditions acknowledging the risks involved in participation.
17.2 If a guest would like to participate in golf and use the Paragolfer, they must:
• Fill in the medical history form which is part of the booking form. This form must be filled out each time a guest stays at Sargood on Collaroy.
• Sign the terms and conditions acknowledging the risks involved in participation
• Have their treating physician fill out the medical clearance form which can be found on our website or is sent as part of the booking process.
• If the guests treating doctor thinks it appropriate, then a current (within the past year) bone density score may be required before participating.
17.3 If a guest would like to access the Gymnasium independently, they must:
• Fill in the medical history form which is part of the booking form. This form must be filled out each time a guest stays at Sargood on Collaroy. The form will be reviewed by Sargood clinical staff and if they think it appropriate the guest may be asked to produce medical clearance from their treating physician before accessing the gym.
• Sign the terms and conditions acknowledging the risks involved in participation.
17.4 If a Guest would like to participate in a Physiotherapy or Exercise Physiology session, they must:
• Fill in the medical history form which is part of the booking form. This form must be filled out each time a guest stays at Sargood on Collaroy.
• Sign the terms and conditions acknowledging the risks involved in participation
• Have their treating physician fill out the medical clearance form which can be found on our website or is sent as part of the booking process.
